Classic Driving School

Instruction Permit Requirements:

Students must present the required documents listed below to the Department of Public Safety to be eligible to receive their instruction permit.

1.  Proof Of Identification:

• U.S. Birth Certificate 
  (original or certified copy)
• Texas Identification Card
• Citizenship Certificate
• Resident Alien Card
• Foreign Passport with Visa Authorization

2.  Original Social Security Card: (must have actual card)

• The names on identification and Social Security card must 
  match.  

3.  DE-964 Form

• Provided by Classic Driving School after first six hours of classroom instruction completed and permit test passed.

4.  TEA Form (Verification Of Enrollment):

• This form is obtained from the student's attendance office  
• From August to May this form only lasts thirty days from the date of issuance.
• During the summer, this form is valid from the date of  issuance to the first day of the  fall semester.

5.  Application (DL-14A):

• This form will be provided by Classic on day 4.
